Exosomes Therapies For Hair Loss
Several new companies are developing exosome therapies to treat hair loss. Unlike previous iterations of exosomes treatments, these therapies will undergo clinical trials within the FDA approval process.
Exosomes And Hair Growth
Around two years ago, exosomes became the latest hot topic in hair restoration and hair loss treatments. Various hair transplant surgeons and dermatologists began offering exosomes injections as a sort of “next generation PRP” treatment. A few impressive hair growth results were featured online, and then things suddenly became quiet. This was, in part, due to the FDA clamping down on the use of exosomes without first conducting proper clinical trials. Exosomes are described as extracellular vesicles which act as the signaling molecules from one cell to another. Some researchers propose that exosomes are, in fact, the greater source of benefit in approaches such as stem cell therapy. The genuine potential of exosomes is still being discovered and luckily there are many companies who are pursuing exosomes as validated therapeutics.
Shelf Stable Exosomes – Rion
Among the most interesting exosome-based companies is Rion LLC of Rochester, Minnesota. Rion was founded by two Mayo Clinic-trained MD-PhDs, Dr. Atta Behfar and Dr. Andre Terzic, respectively. Both physicians have an extensive background in cardiovascular and regenerative medicine. Rion’s platform therapeutic, Purified Exosome Product or PEP, is the result of its founders’ efforts to create an optimal therapeutic product for soft tissue regeneration. PEP is derived from human platelets. The Rion website describes PEP as “the world’s first truly off-the-shelf allogeneic regenerative medicine platform….designed to promote cell growth and formation of new blood vessels, while also reducing inflammation and protecting cells.” An important novelty of PEP lays in its ability to remain shelf stable. Pending its performance in clinical trials, such as an active trial in wound healing, PEP would have many intriguing medical applications, including aesthetics.
Rion’s pipeline currently shows indications for myocardial infarction, wound healing, urinary incontinence, joint disease, and an injectable hair loss treatment. Coincidentally, Rion has spun out a topical skincare product from PEP technology which is currently available, and recently published results from a six week clinical trial. We hope Rion begins its program for hair soon and look forward to sharing updates on that front as news becomes available.
Exosomes Containing miR-218-5p – Xollent Biotech
One of the most famous studies involving exosomes and hair growth was completed by Dr. Ke Cheng at North Carolina State University. The peer-reviewed article identified microRNA (not to be confused with messenger RNA or mRNA) 218-5p as an important factor in the hair growth potential of exosomes. Dr. Cheng and his team found that exosomes derived from 3D cultured dermal papilla cells contained an upregulated expression of miR-218-5p compared to cells cultured in 2D. The study was published in July 2020 and for some reason multiple people have been sending me this study over the past month to review. After further digging, and to my pleasant surprise, I discovered that that Dr. Cheng has taken the initiative to further his research in exosomes and microRNAs into a clinical setting.
You will notice a program for alopecia in the Xollent pipeline involving exosomes in a spray application. It would still need to be verified by Xollent Biotech, but it seems quite possible that a spray treatment within a cosmetology bracket would have a potential abbreviated route to market. In any event, the program appears to be in early stages. Stay tuned for further updates.
Targeted Therapeutic Exosomes
If you’d like to enjoy learning about further therapeutic applications of exosomes and their technological advancement, check out the ILIAS Biologics company. Ilias does not currently show any active programs in hair, though, they are one of the most fascinating exosome companies I discovered during my research. Ilias’ platform technology describes potentials of loading exosomes with cargo (specific molecules, etc.) and modifying the surface of exosomes for enhanced target delivery.

It doesn’t look like any of these have a solution anywhere close to entering clinical trials. Very frustrating. Hair loss is the biggest untapped commercial market on the planet yet these companies screw around on all these other things. Hair loss revenue would fund everything else they could want to do if they solve it.
I’m not sure if it’s always companies “screwing around.” Sometimes that could be the case, but much more often it’s a lack of funding.
The spray is very interesting. From what I’ve looked up on Exosomes I have yet to see any great results the ones I found very similar to that of PRP but I’ll remain hopeful. Also stacked with AR inhibitor or DHT and other meds it could be good for those that still have a bit of hair to rescue 🙂

Many claim in forums that the hmi-115 would probably become extremely expensive. However, on the Hopemedinc website it says:

Ben, I’d say your options are a bit limited without min or fin and living in Poland. You could take up microneedling I suppose. There’s also ketoconazole shampoo, but I really believe it would have a quite limited effect as a solo treatment. As you may have seen in an article, a product called CosmeRNA which published studies in a Nature related journal, is aiming at a release as soon as possible. The results are modest, but I’ve seen a lot of people online have interest in using it as a maintenance treatment. To the best of my knowledge that product should be out in EU in early 2023. Beyond that Kintor KX-826 is the next best hope.

Jason, I will be able to get clarity on that situation for sure, but it may take a little while as I’m currently waiting to hear back from the company. In the meantime, my guess is they are still pursuing it because the cosmerna link is still on their website. Cosmerna seems to be a product name specifically for cosmeceuticals.

I have inside information on cosmerna from a person who works there . He told me that the company was rejected for a cosmetical which makes sense since it’s an AR silencer so they will not be a cosmetic . But now they are using a higher dose and taking it to clinical trials so if all goes well the higher dose could be potentially better but the downfall is more and more time . Hey so in our group buys people have been using 99% purity pyri/kx for 3-6 months it’s been tested same solution and everything and people have been reporting sides and no maintence or improvement . Personally, I think it’s a slight risk for people to buy new drugs from third party manufacturers, especially before completion of clinical trials. This is mostly common sense right? That said, I understand why they do it. I think their feedback on the drugs stands for something, but it also has to be taken with a grain of salt since they’re not getting the drug from the official source.

Just checking in. Not a whole lot to write home about in pas year or so but just wanted to provide some confidence pump ups! Hair loss used to bother me alot more when I was younger. I’m 30 now about norwood 5 maybe 4. I’m possibly looking at hair transplant situation but honestly a bit of a back burner issue in my life right now.
Most people care about this because of women. Yes obviously we all know it would be fantastic to have a beautiful hair but seriously I remember being bent out of shape about this years ago.. just wanna say don’t focus on it too much gents.. it is what it is. Focus on what you can control. Ie. Gym, business, career, sense of humor..

Olumiant is a drug for alopecia areata which is distinct from common hair loss. Olumiant will not be a useful or practical treatment for pattern hair loss.
I thought Amplifica would do the clinic test (phase I) this year.
I believe Amplifica has raised enough money to get into a trial, I think you’re referencing the comment made by Dr. Rassman on Reddit? It’s just not clear whether their first drug is Scube3 or if Scube3 is something new. In any event, they are still raising money and can always use more funding to expand and hasten their development (like every other company). They have multiple drugs in their pipeline.

Chris, Shiseido is doing a clinical trial to observe if the effects of the DSC cell therapy over multiple injections warrants a product. It’s not completed yet, but when they do complete and release info it will be easy to find on this site.

Hi Lotus, thank you for the kind words. The only thing I could say is there is not much consistent hard evidence that exosomes work to treat hair loss and cause regrowth. Some people have reported benefit, and some clinics will show one or 2 examples of successful patients and then market exosome treatments really hard, but that is not validation. Previously, the FDA began to admonish clinics from injecting exosomes without regulatory approval as well. That’s all I could share with you, best of luck with your decision.
